Consultation location: 5916 Lake Michigan Dr, Allendale, MI 49401.

Infinity Dental is responsible for patient care, records and follow-up.

This Notice explains how we handle your information before and while you are connected with the Treating Practice: when you see our ads, visit our consultation pages, use our chat assistant, fill out an intake form, or receive our texts, emails, and calls.

2. HIPAA and the Treating Practice

The Treating Practice is responsible for your clinical records, including dental and medical history, scans, X-rays, photos, diagnoses, treatment plans, and billing records. HIPAA protection depends on who holds the information and the capacity in which it is handled; it does not depend solely on whether you have become a patient.

When BFC Management LLC, operating as Allendale Dental Institute, collects or handles protected health information on behalf of the Treating Practice, it acts as the practice's business associate under a written business associate agreement. We use and share that information only as permitted by that agreement and HIPAA to support the Treating Practice, including intake, contacting you and scheduling, or as otherwise permitted or required by law. This Notice describes our intake and administrative role. Infinity Dental remains responsible for clinical care, dental records and follow-up.

3. About the free consultation

Free dental implant consultations and clinically indicated X-rays are presented by Allendale Dental Institute. Care is provided by Dr. Jared Van Ittersum and Dr. Elias Achey of Infinity Dental. X-rays are provided only when the treating clinician determines they are clinically indicated. There is no obligation to proceed with treatment. Any treatment you choose afterward is scheduled and priced separately. The consultation and clinically indicated X-rays are free; no insurance plan or other third party is billed for these services.

4. Information we collect

Information you give us

  • Contact details: name, phone number, email address, ZIP code or address, and preferred location and appointment time.
  • Basic dental interest: for example, whether you have missing teeth, wear dentures, or are interested in dental implants or full-arch treatment, and how you heard about us.
  • Financing interest: whether you would like to see if you prequalify for patient financing.
  • Messages: what you type into our chat assistant, texts, or emails, and what you tell us on the phone.

We ask for only what is needed to schedule you. Please do not send detailed medical history, medications, insurance numbers, or Social Security numbers through our forms, chat, or texts; the Treating Practice will collect what it needs through its own secure patient forms.

Information collected automatically

  • Website and device data: IP address, browser and device type, pages viewed, the ad or QR code that brought you to us, and approximate location derived from your IP address.
  • Cookies and similar tools: Google, Meta, and iHeartRadio Magellan technologies are used across our website. See Section 8.
  • Call details: if you call a tracked number, the number you called from, the time and length of the call, and which ad or mailer led to the call. Calls may be recorded; when they are, we will tell you at the start of the call.

Information from other sources

  • Advertising platforms: if you submit a lead form inside Facebook or Instagram (Meta) or another platform, that platform sends us what you entered. Meta collects that information under its own privacy policy and knows you responded to our ad.
  • Mailing lists: we obtain names and mailing addresses, and general household estimates such as age range, estimated income, or home value, from commercial data providers so we can mail postcards about free consultations. This information is not health information, and we do not receive your credit report.
  • The Treating Practice: may tell us whether you scheduled or attended, so we can stop sending you reminders and measure our results in aggregate.

5. How we use your information

  • Contact you about the consultation you asked for, schedule and confirm your appointment, and send reminders and directions.
  • Send your request to the Treating Practice so it can contact you and provide care.
  • If you ask, connect you with a financing partner for a prequalification.
  • Answer your questions through chat, text, email, or phone.
  • With your consent, send you information about future consultations, events, and offers from the Treating Practice and Allendale Dental Institute.
  • Measure, in aggregate, which mailers, pages, and ads lead to appointments, using the safeguards in Section 8.
  • Keep our systems secure, prevent fraud, and comply with the law.

We do not use your health-related information to decide your eligibility for credit, insurance, or employment. Website tracking and advertising measurement are described in Section 8.

6. How we share your information

We share information as described below, including website tracking information with the providers identified in Section 8. We do not sell your dental records.

  • With the Treating Practice: your intake information goes to the practice you selected (or the practice serving your location) so it can contact you, schedule you, and treat you.
  • With a financing partner, only if you ask: if you request financing information and select a financing partner, we may send your name and contact details to that partner so it can invite you to apply or complete a prequalification. Listing financing options does not authorize us to send your information to every listed company. The partner explains its own process, including any credit check, and its own privacy policy and terms apply. We do not receive your credit report.

Financing options include, but are not limited to

  • CareCredit
  • Proceed Finance
  • Lending Club
  • Sunbit
  • Alphaeon
  • PatientFi
  • Cherry

For more information about financing, contact us at hello@allendaledentalinstitute.com. Availability and financing terms are determined by the Treating Practice and the selected financing provider.

  • With service providers: companies that work for us under contracts limiting their use of your information to providing services to us, such as CRM and texting (GoHighLevel), forms and chat, call tracking, scheduling, teledentistry video, website hosting, and email. Our current service providers for intake are: GoHighLevel (CRM, forms, texting).
  • With website analytics and advertising providers: Google, Meta, and iHeartRadio Magellan receive website tracking information through the technologies described in Section 8.
  • Legal, safety and business transfers: when required by law or legal process; to protect rights, property, or safety; with our professional advisers; and in connection with a merger, financing, or sale of assets, subject to this Notice.
  • With your consent: in any other case where you ask us to or agree.

7. Text messages, calls and email

If you agree, Allendale Dental Institute and the Treating Practice may call and text you, including with automated technology and prerecorded messages, about your consultation, appointment reminders, and offers. Consent to marketing calls and texts is not a condition of any purchase, consultation, or service.

Message frequency varies. Message and data rates may apply. Reply STOP to cancel at any time. Reply HELP for help, or email hello@allendaledentalinstitute.com. Carriers are not liable for delayed or undelivered messages.

No mobile information will be shared with third parties or affiliates for their marketing or promotional purposes. Text-messaging opt-in data and consent are not shared with any third party, except our service providers that deliver messages and the Treating Practice that is providing your care.

You can also opt out of calls by telling the caller, and out of email by using the unsubscribe link. We will honor opt-out requests made by any reasonable means within the time the law requires. Our texts will not include detailed health information.

8. Cookies, pixels and advertising tools

We use Google and Meta pixels and iHeartRadio Magellan tracking technologies across our website to understand traffic, measure advertising and attribute visits or responses to campaigns. This description applies throughout the website, including consultation and intake pages.

  • Information collected: depending on their configuration, these technologies may collect IP addresses, cookie or device identifiers, browser and device information, page URLs and page views, referral sources, timestamps and website interactions or conversion events. A page URL or interaction may reveal an interest in dental services.
  • How tracking information is used: Google, Meta, and iHeartRadio Magellan may process the information they receive to provide analytics, campaign attribution and advertising measurement. Their own privacy policies also describe their processing.
  • Health and intake information: we do not upload intake information or patient information to advertising platforms, including for audience targeting or retargeting. Our handling of protected health information remains subject to the Treating Practice's business associate agreement and HIPAA. This Notice and ordinary cookie consent do not authorize disclosures of protected health information that require a separate HIPAA authorization.
  • Your choices: you can block or delete cookies in your browser and adjust advertising preferences through Google and Meta. Browser settings may not stop every tracking technology. We honor Global Privacy Control (GPC) opt-out signals and requests to opt out. To make a request or ask about website tracking, email hello@allendaledentalinstitute.com.

9. Consent text for intake forms

These checkboxes appear on our intake forms and are unchecked by default. Only the first is required to submit a request:

  • ☐ Required: I agree that Allendale Dental Institute may send the information I submit to Infinity Dental so the practice can contact me, schedule my free consultation, and provide care. I have read the Patient Intake Privacy Notice.
  • ☐ Optional calls and texts: I agree to receive calls and text messages, including marketing messages and messages sent using automated technology or a prerecorded voice, from Allendale Dental Institute and Infinity Dental at the number I provided, about my consultation, appointments, and offers. Consent is not a condition of any purchase or service. Message frequency varies. Msg & data rates may apply. Reply STOP to cancel, HELP for help.
  • ☐ Optional email: Email me about my consultation and future events and offers. I can unsubscribe at any time.
  • ☐ Optional financing pre-screen: I'd like information about patient financing. I agree that Allendale Dental Institute may send my name, phone, and email to the financing partner I select so it can contact me. The selected partner's own terms and privacy policy apply to my application.

10. How long we keep information

  • Intake and lead records: we retain these records for seven years after your last contact with us, then delete or de-identify them, unless a longer period is required by law or a legal hold applies. The Treating Practice retains its own clinical records under its policies and applicable law.
  • Records of consent and opt-outs: we retain these records for seven years after the recorded consent or opt-out, subject to any longer legal retention requirement. We preserve the minimum information needed to continue honoring an opt-out.
  • Call recordings: we retain recordings for seven years after the call, subject to any longer legal retention requirement or legal hold.
  • Chat transcripts: we retain transcripts for seven years after the conversation, subject to any longer legal retention requirement or legal hold. Other identifiable information we retain under this Notice is kept for seven years after the relevant interaction, subject to the same exceptions. Tracking providers may retain their own copies under their policies and applicable agreements.

11. Security

We use reasonable administrative, technical, and physical safeguards designed to protect your information, including access controls, encrypted connections, and limiting access to staff and service providers who need it. No system is perfectly secure. If a breach affects your information, we will notify you and others as the law requires.

12. Your privacy rights, including health data laws

HIPAA rights: to exercise applicable rights to access, amend or receive an accounting of disclosures of your protected health information held by the Treating Practice, contact us at hello@allendaledentalinstitute.com so we can direct your request to Infinity Dental.

State privacy rights: depending on where you live and which laws apply, you may have rights to access, correct, delete or obtain a copy of your personal information; opt out of targeted advertising, sale or certain profiling; give or withdraw consent for sensitive data processing; and appeal a decision about your request. These rights are subject to applicable exemptions and legal retention requirements.

How to make a request: email hello@allendaledentalinstitute.com. We will verify your identity by matching information you give us to what we hold, and respond within the time the law requires (generally 45 days). You may use an authorized agent where the law allows. We will not discriminate against you for using your rights.

13. Children
